We the undersigned, request the enrollment of the above student in accordance with the terms and conditions specified in the Waiver Document and with the Social Rules outlined in the Brochure of SGIS, a copy of which has been read and understood. We understand that the school records of the student will not be released unless we have given full payment of all school fees.
